Understanding Care Management Reqs and Medicaid Status Impact Re HCBS Eligible Children

The state offered a webinar on Understanding Care Management Requirements and Medicaid Status Impact Regarding HCBS Eligible Children/Youth.  Recent guidance was reviewed and explained to assist Health Home, C-YES and Medicaid Managed Care Plan care managers in recognizing the requirements necessary to continue HCBS eligibility for a child/youth and how their Medicaid status might play a role.

Children's HCBS Capacity Management Refresher

This webinar provided an update and reminder regarding the Statewide Capacity Management processes for the Home and Community Based Services (HCBS) Children’s Waiver and the interaction needed with Health Home care managers.  The training highlighted how important it is for the Health Home care managers to communicate with Capacity Management and ensure that children who are HCBS eligible and receiving services have a waiver slot assigned.  Additionally, Capacity Management needs to be notified when changes occur to a child’s status so an unnecessary waitlist does not occur or if a waitlist

Children's Health and Behavioral Health System Transformation in NYS: Presentation for Children and Families

NYS recorded a webinar on the Children’s Medicaid System Transformation in NYS in order to present this information to children and families. 

This webinar provided an opportunity for children and families to learn about changes happening to the Children’s Medicaid System and what that means for them, including additional opportunities coming their way.

Children's Aligned HCBS Authorization & Payment for Adaptive/Assistive Equipment, Veh. Mods, Environmental Mods, NMT

This webinar reviewed the process for a child to receive an Environmental Modification, Vehicle Modification, Adaptive and Assistive Technology, and Non-Medical Transportation.
The webinar outlined the key roles of the process, required documentation and payments process for all four services.

Children's HCBS LOC Eligibility Determination and Documentation Review

The Department of Health reviewed how to conduct a HCBS/LOC Eligibility Determination and the necessary documentation. 
Beginning April 1, 2019, children seeking HCBS can only be determined eligible under the new Children’s Waiver by Health Home care managers, the Independent Entity C-YES or the DDRO.

Target audience:  Children service providers inclusive of Health Homes, Health Home Care Management Agencies,C-YES, DDROs, Waiver Providers, etc. 

Children's HCBS Planning and Plan of Care Development

This webinar reviewed person-centered care planning and the creation of a Plan of Care for the consolidated Children’s Home and Community Based Services (HCBS) Waiver. Presenters discussed the steps needed to engage families and briefly reviewed the children’s HCBS workflow. This webinar also covered the various target populations, how/when to determine which HCBS will be most effective for the child, and making appropriate referrals.

NYS Children's Medicaid System Transformation: A Discussion with Children and Families

Children's HCBS Capacity Management

This webinar addressed the Statewide Capacity Management Process beginning April 1, 2019. This training covered how capacity will be managed for the consolidated 1915c HCBS Children’s Waiver and the interactive process between the Health Home care managers, the DDRO and C-YES with the DOH Capacity Management Team.  Once HCBS Level of Care has been determined, the Health Home care managers, the DDRO and C-YES must receive confirmation from the DOH Capacity Management Team that a slot is available.
